An internally most handsome example of this exceedingly uncommon item (cited as entry sixteen on pages 66-67 of Giorgio Maffei's "Allan Kaprow: A Bibliography") additionally BOLDLY INSCRIBED "For Sherry & Mel / Allan" in red ink on the front cover showing some soiling and typical minor age-patination to its white wrappers.
